KELLEY-ROSS PHARMACY GROUP WINS PRESTIGIOUS QUALIS HEALTH AWARD
FIRST COMMUNITY PHARMACY TO WIN ‘AWARD OF EXCELLENCE’
SEATTLE, Wash. – May 18, 2017 – Qualis Health recently announced Kelley-Ross Pharmacy Group as the winner of its prestigious Award of Excellence: Pharmacy. The first community pharmacy to ever win this statewide award, Kelley-Ross was recognized for a joint program it developed in partnership with Virginia Mason Medical Center’s Cardiology & Cardiothoracic Surgery.

The joint program – Heart 2 Heart: Improving Outcomes and Decreasing Readmissions for Heart Failure Patients through Integrated Pharmacist Home Visit Model – focused on pharmacists home visits with patients who had moderate-to-severe heart failure. For three months following discharge, Kelley-Ross’s Clinical Pharmacy Institute (CPI) led monthly home visits with heart patients in addition to regular phone check-ins and follow-up visits with care providers at the Virginia Mason Heart Failure Clinic.
The national readmission rate for these kinds of patients is 24 percent. After three months collaborating on in-home pharmacist visits and , the Kelley-Ross pharmacists helped reduce Virginia Mason’s readmissions from 21 percent to 8 percent.
