To help do this, we partner with the Polyclinic Community Health Foundation and their PharmaConnect Program to provide assistance when you need it.

Long-term medications are provided on an ongoing basis by connecting you with our pharmacy assistance coordinator, who guides you through the (confidential) application process of finding the appropriate pharmaceutical industry based program.

Since its inception in 2005, the PharmaConnect program has helped over 400 patients receive medication they need to feel well enough to return to work, care for their families or manage a chronic illness.
